Got this item with ok drop rate in Sewers of Nihilia, the Crematory.
Too bad I visited this one last, drop rate was awful in other Sewers expeditions...

I just got the Writhing Mass of Insect Larva off of a muck-covered fly in the Sewers of Nihilia, Pool of Sludge experience instance.

just an fyi if anyone is having problems getting this drop:
I was boxing in the 'plant' missions with my druid + cleric, spent a total of days and days (seriously, way way way too much time!) just killing all the mobs in the first room of the plant instances, no luck at all (though i did get about 20 'globe of mud's to drop...)
I got frustrated tonight, dropped exepdition, got the 'lair' mission, and guess what? The **very first** mob I killed ("a muck covered fly") had this drop, the larva.
All I can say is, if you're stuck on this drop, make sure to try the Lair mission too.

I found one of these on A Diseased Gnat right at the zone in of Purifying Plant.
