If he's wandering the undead hallways in the NW corner of zone he may be in the basement portion. This is accessed thru the trap door found on the west side of the corridors (should be marked on your map.) I wasted a good half hour trying to find him via tracking in the upper corridors but as soon as I fell down this trap door HE found ME.

PHs are the "a dream phantom illusion" mobs in the zone's northwestern halls (one spawn is on the first room on the right upon entering the halls, and the other three are roaming spawns in the basement of this area).
It hits for a max ~14,000; and has ~1,000,000 hitpoints.

There is a ph upstairs in the undead area as well. when you get to the top of the stairs the first door on the right has a ph in that room.
Spawns amongst the undead in the west maze area, north of leps.
