How does the effect % work in to total HP healed?
Is it [base * (1+aa's+effects)]
[base * (1+aa's) * (1+effects)]
or [base * (1+effects) * (1+aa's)] ?

All of your bonuses are added together, before they are applied to your base. The best of each type of bonus is used if there is a stacking question (i.e. only one effect from an item).

I hate this thing....well ok I hate the 4 chain and one silk bracers that have fallen and still haven't seen 1 of these drop yet.

Wee, my cleric can crit with melee wearing this. Highest so far was 192 vs an undead with the summoned hammer of damnation.
Does this work on Sk's lifetaps?
The focus DOES work on SK and Necro heals. I had it on my Necro and you can see the focus happen and the difference in the recourse heal.

actually yes the focus should work for the healing portion of the lifetap as it helps with my lifetap procs.
No improved damage works on SK lifetaps.

The healing focus has no effect on the heal portion of SK/Necro taps, because it is basically an instant detrimental spell first, and then a recourse spell that heals. The AA skill Theft of Life permits us to get exceptional heals on taps, however.
Lifetaps are only focused by MR or all-resist inclusive DD focus effects.

Won the roll on this tonight. Pally rolled 96 I rolled 97. Very happy with the increased heal even if it is a chance of 1% to 25 % heal increase for my cleric. Stats where great upgrade for me too. We took down Kyv Heartstriker Jhiru with a cleric, chanter, druid, pally, and war. Other cleric went LD. Tipt flag was loads of fun and im really liking GoD!

I just got this tonight. Must say, it's very nice. I'm noticing clusters of triple crits against monsters. Cleave is likely my favorite effect now. Hope this translates into more slays for me against undead.
