Just to confirm, he will not spawn at the 3 guys out by the chasm, nor the 5 by the false bridge. Cleared them at least 30-40 times on a bonus-name server during bonus-name week. Went inside the tunnel, just past a few shriekers and there are a group of 4 and one nearer the shriekers. It was the solo one that popped for me after 2 clears.

This NPC (a hideous thought leech) spawned at in-game loc -510, +438, -64 approximately. This is just inside the mouth of the south-west tunnel on the east side of the zone that exits out to the zone's central chasm.
