Not sure if this is how it's done, but here is what happened for me.
I was working my way back to Shei Vinitras and was exploring inside the room to the north of Shei's room (farthest room north on the map) when I saw this emote:

Immediately thereafter, The Insanity Crawler was on track.
I speculate there was a hidden trigger in the room. Will it always be in that room? No idea.
This appears to be bugged now. I triggered it, it cast an AE and then disappeared before it even showed up on my target. This happened about 5 minutes ago.

Killable at zone line. Killed it for the Luclinite orb with one grp. One druid pulled it to zone, chanter mezzed adds, one paladin tanked it, other druid healed, necro dotted and used pet and the last Paladin did something, not sure what. Easy to kill, dropped KEI and Erjav's Lost Competition, Belt and Shield.
Can confirm that if you sit out of the AE range the mana drain does not land. If someone dies the worms do indeed spawn. The first druid was the unhappy victim, but we killed off the mini worms with little fuss. Go go Pally rez.
Killed him tonight with a small group of 5 and myself outside the group. I just happend to be there and offered to help. We killed him in less than 5 mins. But he died along the zone out from Ahkeva to ME and we could not loot him. Before you could loot you would zone out to ME when you got near the corpse. Been 90 minutes since we petitioned and still no answer. Lesson: Don't kill him on the zone line.
Killed him today with 9 guildies: 65 brd, 65 shm, 60 clr, 58 mag, 65 war, 65 wiz, 65 dru, 63 sk, 62 rng. He dropped the shield, belt and Orb of Luclinite. If there is a manual way to trigger him, I don't know how, since all we did was kill the mindworms to spawn him. However, I doubt there is a way to trigger him, since he seems to be a random and somewhat rare spawn. I've killed mindworms for hours before and no Insanity Crawler ever popped. He killed one person on the first try, and 3 or 4 mini-mind worms spawned so we evaced and redid him. He summons just like all the mindworms do. His health regeneration is very slow and his health was still less than 100pct when returned to camp. He spawns to the left of the centi hallway, the one just before where all the mindworms are. I would recommend killing him in this hallway away from the mindworms, so that you only have to deal with the insanity crawler summoning you. It also makes it easier in the Centi hallway because the Centis are easier to handle than any mindworm spawns. Before pulling him again, we cleared the hallway of all centis and had our SK tank any adds that came or repopped. Casters stayed at the far end of the hallway away from its manadrain and casted at max range. Healers would only run within healing range when necessary and then get back out of range of the Insanity Crawler's spell. He was easily killed with the force we had and slow landed on him without too much trouble. No one died when we killed him, so no miniworms spawned. I don't think it gates, but we had our casters nuke him when he was low on health just to be safe. Hope this info helps.
I killed this guy with 4 people 1 lvl 65 cleric 62 shaman 65 paladin and a 65 druid...just keep the casters behinde the corner pop in to cast spells...its cake once slowed 75%..

Curious why everyone is convinced this guy is huge... I was checking Va'dyn spawn once and triggered the trap for this guy, FDd right away, F9d a few times and took a look around. Hes about the same size as the worms in Maidens Eye. The only thing I can figure is that there is another, bigger, insanity crawler, or people are getting this confused with other big worms in other zones (like Acrylia or The Deep). The first explination seems more likely because I cant imagine so many people getting zones mixed up.
Just a note on the ae mana drain, at first it seemed like it was a big mana drain but after doing some searching i've found its only 200 mana each time, but it does proc very often and seems to be unresistable.

Best strategy is stay away from him, get an extended range item, and if you need to get within his ae range to heal, nuke, etc. make sure you move back after that.
