On Al'kabor, this item has dropped off of "a grimling high commander" which no longer spawns PC side.
Sadly a craptastic piece of loot from such a nasty encounter.

Its also a mob, now, as of the revision to Grimling. Specifically, it spawns near/at the end of (I believe) the 9th Grimling raid.

Nope, he is the final named mob in the 10th raid which is the grimling town raid. You must complete all the other 9 raids (the 9 camp raids can be done in any order) before you can attempt the town raid.

I'm thinkin that I'm going to copy that picture and edit it in MSPaint... just so I don't need to scroll up, down and over to see the whole thing. I'll submit it later. Heh.
