I did this today with 3 Sapphires - so four is incorrect.

this can be made by chanters with 2 sapphire and 1 poison vial. with spell distill mana

Enchanter's make this using distill mana (lvl 38) spell, 2x sapphires and a poison vial.
Mass distill mana spell (lvl 39) uses 10x sapphires and 5x poison vials.
