It's a reward for doing all of the Challenger achievements in the Empires of Kunark Expansion. I've added the achievement/quest and linked the item.

I'm guessing this is a Empires of Kunark item by the item name and when it showed up during a specific site update for us. Similar to the Obulus Death Shroud in Old Seb.
Since Empires of Kunark doesn't have an Old Seb zone, it probably drops in one of the other zones.
