I got this to spawn today while doing the Hunter achievements. It did give the prompted text other users have reported. Saying "I will cleanse the rage spirit" spawned "an enraged luggald abomination" which seemed no different than the many other trash mobs in the zone, and like others reported, dropped no loot.

Emote upon spawning "An ancient spirit stands over the glowing pool of magical power."

i can confirm he does exist and says "i sense a young spirit of flesh and blood nearby. Those that wish to corrupt it have trapped the spirits of four fearsome beasts within this magical pool. They are known as the frenzy, berzerker, fury, and rage spirits. Tell me which of these do you choose to cleanse from the pool? Be forewarned these are powerful creatures that can rarely be matched in battle." i said frenzy and got "a frenzied luggald abomination" didnt drop anything though .

He's in the non-mining area of the zone (Northern Half). If in your exploration, you come across a room with 5 Luggalds surrounding a small pool of water, kill those luggalds and he may spawn. Hail the water spirit and he will tell you that you can choose to help cleanse the area of something and gives you four unbracketed choices:

rage
berserk
other
other

(Memory is foggy, I have the screen shot at home)

Either way, just say one of the options and a taller luggald will spawn with an associated name. (An Enraged Luggald, etc)It will aggro you, so you may want to defend yourself. I've never had one drop anything.

The water spirit despawns when you make your choice.
