You lose factions with the "good guys" because you have to kill their friends to get the heads you need to finish the quest and you get better faction with "bad guys" because those factions are in direct opposition to the factions the NPCs you kill are on and they want them dead, too, so they reward you with better faction for killing their enemy. Sword is kind of middle of the road, the weight really kills it even with the STR buff. Probably not worth all the faction hits to get it because you could become KOS in one of the light cities and the dark faction hits may not be enough to raise you up from being KOS in those places either, so you could end up a homeless wanderer. :(

What I dont get is you lose faction with the good guys when you do this quest! Stupid verant! Anyways if you've already done this quest, no problem, just goto Highkeep (lvl 20+) and kill Osargen, if you dont know where he is, dont worry someone will show you no problem, hes pretty easy to find and good exp for lvl 25-30, fairly easy, if you've never been there before, dont worry about anything killing you, unless your afraid of little old prisoners which are lvl 10-12 and no faction lose. I forget the negative factions but you definatrly gain faction with Knights of Truth and lose factoin with bad factions.

I agree that the sword is not a wonderful item at lvl 40, however for a lower level this sword is not that bad.
This thing is heavy, and has decent dmg/dly and a +5str (which is kinda cool). The enduring breath will be useful someday when you chase down the plagued shark. Although I would suggest doing so with a better weapon in hand :)
I think I will go earn this sword just for giggles. I will add it to my trophy case :)

check out that weight!!!! 12.5 dang thats pretty heavy for a 1hs... i think this weapon should get an upgrade like unlimited charges of enduring breath! This is the same lvl as the ghoulbane and probably a bit harder to get, so shouldn't it have a little nicer stats and may be a better proc on it? I have never seen someone use these before probably because its not worth running all over the world killing npcs(and their faction) With all the new dungeons and such getting new and better gear all the quests should be looked over again and either lower the lvl of the quest or better the stats of the reward.
if you look at it this way: it weighs 12.5 but has +5str and if you think about it that increaces the amount you can carry thus making it only weigh 7.5stones (12.5-5=7.5) well thats just my 2cp
But that +5 STR only counts if you have the weapon in your primary hand. If, like me, you carry several weapons around with you to keep all of your skills up then the 12.5 weight is a killer when it is in one of your bags especially if you are a high elf or erudite paladin with low STR to start with.

At the time you can get this sword, you can camp the Shin Lord in Guk to get the Ghoulbane a MUCH BETTER paladin-only sword : dmg15, dly34, weight less(can't remember, but less than 12.5, which is a terrible stamina-sucker), proc : dismiss undead, a 100+ DD vs undead. Needless to say, don't bother getting this cutlass that would kill some of your highly valuable factions (like the Kights of Truth in FP).
