Level for effect is level 1, since that's the level you have to be to get the mush and turn it in =p
the mushroom is located behind the guy that gives Gloomingdeep Blessing (and gives you a spell if you are a caster) and are stackable. You can turn in for the vial, which is level 1, level 1 is all ya need.

yes, but on this item, they mean that it is being cast as if by a level nine character, so you are getting a better effect than if it used your level as casting level.
