This is found in Karnor Castle (ground spawn), will try to explain where.
When you in front of the Hands room, you go left and stick left until you get in a little room with a square cube in the center, looking like some kind of altar. The sword spawns on it. No clue or repop time.
The room appeared to be safe, but probably some roamers walking by.
