I have killed him about 20 times and last night was the first night it dropped. I got the lava engraved choker so many times. I went to the bazaar to see what it was selling 4 and was suprised to see no one was selling it and the bazaar was at it's 500 people max capacity, so this earring is very rare. at least on cazic it is.

It is possible that he spawns twice in a row from the corpses of the regular elementals. I was camping them last night and he spawned from the first elemental I killed. I root/dotted him to death and looted the dru/clr/shm neck he drops. Then I moved on to the next regular elemental and was surprised when he spawned from that elemental's corpse as well. Took a little longer to kill him as I was very LOM, but I root/dotted him again and he dropped the earring that time. I've killed him about 12 times total and have only gotten the earring to drop once. People on Prexus loved seeing me in zone because they knew there would be an /ooc for FCFS on a dru/shm/clr neck piece soon enough.
Noticed this dude spawns about every 3 hours, met him each time my kei was fading, and got ear drop today after 2h55 killing elementals at zoneline.

Your timer theory seems to be about right. It is a random pop off any of the elementals ( including the ones under the lava) and seems to average 1 every 2 or 3 hours. I have never seen them twice in one sitting however. When I camp this I go with a fresh KEI and hold camp til its gone. He has Popped EVERY time ive camped there. I have killed him about 50 times and got mebb 8 earrings. Incidently... on 7th hammer I sell them regularly for 2k although I see vendorbeasts selling them as high as 4k.

Dropd off the Lava Etched Doombringer in Crypt of Nadox. To spawn him, you have to kill the fire elementals at the Gunthak zone line. From what I can tell he is a random spawn that will pop immediately after killing an elemental, on the elementals corpse.
The first time i spawned him I'd been there for 30 minutes, the second time it took 4 hours. He dropped this earring both times, along with a few plat.
Earring sold for 4k easily.
