Saevus Lapis Operarius (click for map image) spawns from PH "an inconstans lapis operarius", in the tower next to the Arx Mentis zone-in: second floor, southeast & northwest rooms.

This creature counts as summoned, so the mage/druid anti-summoned line of nukes will work a treat!
Two spawns, one has a built-in add alongside. I'd advise breaking the spawn if you want to get that one to pop.
Killed first time though that Ocean Crush nuke he casts could be Russian Roulette if it gets you at the wrong moment.
