In Oceangreen Village: Part of the anniversary task "Music for a Weary Soul" (see this quest entry for information).
In Shard's Landing: Part of the task "Sleepless Nights" (see this quest entry for information).
In the Ethernere Tainted West Karana:
"a vengeful spirit" (click for map image) has a chance to spawn after the fifth step is completed in the heroic adventure "Life After Death". It hits for a reported max ~16,000 and has ~5.5 million hitpoints.

Hy .. As I am doin this task for about a week now, i am very used to it. Speakin of this NPC there should be mentioned, that if he DOESN'T spawn, it spawns instead "a spirit". Different cons to me as lvl 84 warrior (white to yellow). There additional accurs no message, if he spawns - other to the vengeful, where a message goes out, that nearby appears a rare creature (something like that).
I didnt kill him often till now, but if you take him to the monsterlist, i will add him to my kill list to get more lootinfos ...

Well.. half correct ... its no PH in the common sense, as if you kill the spirit, never ever the vengeful will spawn. There is only ONE spawn.
Its just EITHER the "a spirit" OR "a vengeful spirit", that spawns after halftime (after talking with the spirit crier). If "a spirit" spawns .. well ... bad day ... if the vengeful one spawns .. grats .. you got the jackpot ) ... either - or.
Have 2 screenshots too .. well .. at least what one can made of an almost invisible spirit .. gg. And also a little loot-list. Can send it to you. Took a look anto the pictures ya have from the others "a spirit"s .. they dont match.
Just tell me how to send them to ya...

I created "a spirit" for this zone so if you want to upload a screenshot for that. You can just attach the loot list to whichever npc in a post.

Dropped https://everquest.allakhazam.com/db/item.html?item=87323 , Shield of Gnollish Rage, at level 86. Killed with level 86 4 box group consisting of Beastlord, Magician, Enchanter, Druid. The Enchanter used group spell Spell Damage Rune, Druid used group heal. Took a few minutes (I did not time).
