Kiss of Death spell was patched to remove Certain Death and Limited Health slots around Aug 20th, 2013.
Killed her today and she cast Kiss of Death 3 times on 3 different targets. Each time the target was feign death and their hit points dropped to near zero (almost like Limited Health effect was still present). Healer merc, one who got kissed, was set to efficient to avoid big heals. There were no deaths.
It's possible Certain Death doesn't trigger anymore.

In Castle Mistmoore (revamp): Has ~7 million hitpoints; casts "Kiss of Death" every 20 seconds; casts "Mocking Reflection" every 30 seconds. If you are above 50% health at the time "Kiss of Deaths" wears off, you are death touched ("Certain Death").

Definitely still triggers. Per Zam, it only DTs if you are above 50% health.

Cast Certain Death (6.4M unresistable nuke) on me today.
She has 7 million hp.
She uses Kiss of Death https://everquest.allakhazam.com/db/spell.html?spell=38762 every 20 seconds. This results in Certain Death https://everquest.allakhazam.com/db/spell.html?spell=38763.
She also used Mocking Reflection https://everquest.allakhazam.com/db/spell.html?spell=38761 after 10 seconds and then again 40 seconds later.
For this fight you will need a mezzer and some good dps. You need to kill her before she kills off your group with Kiss of Death. We ended up having 4 adds with her that the ench locked down with mez.
