This is the main encounter in the raid event "The Beast Below" (see this quest entry for event information). A chest spawns with loot after this event.
Grunkuck also appears in the "Tasnise Underbelly" group mission, but isn't attackable and cannot be targeted.

Spells (particularly slows in my case) tend to land much much more effectively if belly cast. I set up pretty much in the middle of the mobs belly..slow lands fairly easily then I'm within range to heal whatever tank we have keeping the beasts attention.
