OK, to answer some questions
1) Name is wrong here. It is simply Icon of Sacrament.
2) Yes it is lore
3) Yes it is no drop
I stumbled on this while doing part of the
paldin soulfire quest. I dont remember seeing
it but the one time in the 3 evenings I spent
inside the castle waiting for the rare MOB
to pop. I have to admit I was not looking for it tho.

This item is found in the jail in Mistmoore Castle. It spawns on the ground in one of the 3 cells. I'm not sure about the respawn time. I hunted there a few hours but didn't see it respawn.

is found in secret room in mistmoore. if u fall down hole in cemetarey go up ramp open door on right it opens other three doors . its in pair of doors on left . their are 3 dark elfs that con dark blue to me at 33 but im a rogue so i didnt have to fight em anyhow. u can get out of room through secret wall on left as u come up ramp . doors only stay open 11seconds so dont let em close on u
