The above Focus states "4: Limit: Instant spells only"

This appears to be incorrect. I have an item with Blessing of Pious on Finger slot and Dragon health on Shoulders. If i cast word Of V - I get the message that my Obsidian Pauldrens (shoulders) feed you with power.

Accordingly I assume that this effects the group as well as single

From my understanding, the focus with the higher level/healing effect takes precedence. This may explain why your shoulderpads are overriding the focus on your ring.
