I've seen this illusion a few times in guild hall. I even had a GM show up in HoS and cast it on me, and different illusions on my raid members. It's kind of a cool illusion of a floating skull with bugs crawling around, in and out of it's mouth. Sorry, I have no info on what item casts this, but you can bet it's a gm only item at this time.
