When i combined these items in medicine bag on Selos I got Tonic of Heat Affinity V instead of Benefit Awareness V? incorrect recipe maybe?

Most likely a bug. The components for Tonic of Heat Affinity V are completely different.

I believe this might be a TLP only recipe. I just tried to confirm the recipe on a live server and received the message "You cannot make this combine because you fail to meet the requirements" I'm a 300 Alchemist with a Mundunugu Medicine Stick and 8/13 Alchemy AAs.
Also, are you sure it's an omens era recipe? Draconic Essence is only reported to drop/be foraged in Dragons of Norrath zones and for some reason off the Dragons in Field of Scale (according to Magelo).
