Loot a "Shroud of the Forgotten" from one of the named mobs in the Tower of Rot or Argin-Hiz, and evolve it to its highest level.
For a list of nameds in the Tower of Rot, along with their map locations, see this quest entry. For a list of nameds in Argin-Hiz, along with their map locations, see this quest entry. Once it reaches its highest level, a reward window pops up with the following options: Embroidered Drape of the Forgotten Minstrel Furred Mantle of the Forgotten Primalist Golden Cape of the Forgotten Illusionist Holy Shawl of the Forgotten Templar Rugged Cloak of the Forgotten Outrider Runed Mantle of the Forgotten Sorcerer Scaled Cloak of the Forgotten Guardian Shifting Cloak of the Forgotten Blackguard Tartan Wrap of the Forgotten Ravager Velvet Drape of the Forgotten Oracle | ||||||||||||||||||||||||||||||||||||||||||||

Master Sage Lowenn in Argin-Hiz dropped today 4/22/15...in addition to normal loot table and Bloodforged Cloth Robe. Thanks!

With the hotfix on 2014-05-29, the cloak "has been temporarily removed from NPCs that drop it".

For what its worth, Shroud of the Forgotten is dropping again. I got mine today (7/28/2014) of the Magma Behemoth in Argin-Hiz, so it appear to drop in tier two zones, not just Tower of Rot.

Just dropped for me also off Magma Behemoth in AH after 5th kill. Chasing aug, he dropped it as well.
