This is a VERY confusing achievement. Kill all the named except Matilda before defeating their minions. As others have said, kill the Archivist BEFORE the musty tomes. You should get the success message. You can then immediately kill Matilda.

I tried to do this many times, and i would not get the achievement, read everything here and couldnt figure it out. My kills matched below, killing only the named first, then the adds, taking off DS to prevent deaths, merc on passive when in battle, etc. I still couldnt get it, until today.
Go slower. I waited for the Head librarian to catch up to me, after i killed Ardull the Watcher, where he spawns.
Im a 120 rog, so i took off my weps, ran in and attacked the first named, waiting a little bit, then killed it, waited again an extra few seconds, and noticed the script was being slow, that may have been the problem.
Ducking helped me win this. Even with no weps, i guess you can riposte mobs to insta death, ducking prevents this.
I got the achievement after killing Archivist Herrdar, confirmed 3/21/22. Before i killed any of his musty tomes. So if Archivist Herrdar dies and you dont get the Who's The boss achievement, Drop the task, and restart.

It took me a minute to understand this quest but for future reference those who see this achievement:
When you play as a solo class have something that will not reposite in your main hand. Use direct damage spells to your mob, and keep your merc on passive if it's a melee, balanced if it's a healer. After that, it's named > minions, named > minions, etc, till you get to Archivist Herrdar, then kill him all the books and volia ACH!

I did this with a group using a mage for all the dmg. There were no damage shields and pet was on focus. Triggered the mobs and sent the pet after the named, Did NO damage to anything except the named until the named was dead for all the mobs. I still did not get this achievement when Archivist Herrdar died, followed by his books. I pondered the achievement text and attacked Head Librarian Matilda until his minions spawned. I killed a couple and then killed Matilda. His second round of spawns were partially still up.
I didn't get the achievement. WTH EQ.
EDIT:
The more I'm thinking about the description of the achievement, it's got me wondering.
A) can anyone confirm this achievement is received BEFORE you engage Head Librarian Matilda?
B) does Head Librarian Matilda have a limit to the number of waves?
If A is true, then seriously, WTH? If A is not true, and B has a limit, then maybe you need to kill all of Matildas waves.
It's also possible Matilda simply can't have adds up when it dies. He's easy enough to instantly kill before any adds appear, and I've never done that on the same instance I managed to kill the other named before their adds.
