Spawned tonight to Shahrazad on Bertox while we were clearing trash mobs at zone in trying to spawn ZI. Absolutely random, doesn't have to be undead or any specific mobs to kill to spawn. Zone-wide emote and a screen shake when he did pop.

Maggotmiser spawns randomly after killing a random number of mobs in the Demi-Plane of Blood.
Maggotmiser shouts, 'For the love of Tserrina . . . I'm trying to rest here. Who or WHAT is responsible for the commotion? More guests of the Master, no doubt. If you had a dispute to settle with one of the residents here, you'd think you could bother them without picking a fight with every other, single, solitary resident of the Keep.' Find Maggotmiser in one of the eastern bedrooms upon entering the keep of Dreadspire. He procs Dark Maggots, a rear-directional AE, and hits for ~6500. we've found that the trigger spawn to Maggotmiser is in the undead room on the way from the zone-in up to the Spire. Generally when we clear that room, Maggot spawns. When we bypass it to save time on the way to Sisters, Maggot doesn't spawn.

He's factionless and the text listed is all the dialogue you get from him except a
when he dies.
There are no other mobs, items or zones involved, he's a straight kill.
Lockout timer is 4.5 days.

Maggotmiser is not a quest or task, just spawns randomly after killing a number of mobs while in the Demiplane of Blood. When you click up to the Wailing Sisters area, up the stairs on the level above you he's in the complex of rooms to the right.
Swirling Bloodspirit is also spawned in this fashion, don't have his spawn text available though.
