Just tracked this mob today for the achievement (I know I'm years late), but he spawned for me at the top of the pyramid at the far right as you look at the ramp.

The potion is non-stackable.
If you face the entrance ramp to Cazic-Thule, this guy spawns at the top of the Temple/Pyramid to your FAR right (B on map below). His placeholders can be any of the Tae Ew mobs.

Just camped this guy for 12 hours trying to get the boots. All five times he appeared he dropped the Lizard Blood Potion, never dropped the boots. Took about seven hours to get it to spawn the first time, then he spawned three times in a row, would guess from that it has a completely random spawn-time and drops the cazic gate potion as it's common.

Would you happen to know which other special mobs spawn on which temples? Like the Tae Ew Crusader who drops the Decayed armor pieces.

See the Bestiary entry for Tae Ew Crusaders. But basically, they can spawn anywhere on the ramp to Cazic-Thule or on any of the Pyramid/Temples OR at the Obelisk with Radiant. They never spawn as wandering mobs.
