In Fear Itself (raid): Part of the "Cazic's Final Stand" raid event (see this quest entry for event and loot information).
In the Heart of Fear: Part of the event "An End to Fear" (see this quest entry for the group version and this quest entry for the raid version).

May be obvious, but for the purpose of completion, dropped Ambiguous Remnant of Survival for me today.

Also Terror casts a spell Oppression of Thule (seemed to be listed as a spell Dread casts). Looks like its a debuff that wont let you regen mana past 50% and hitpoints past 80%. Its duration is unlimited and cannot remove it with cures, zoning etc. Only own death or killing Terror removed the debuff instantly.

Terror's Ph is one of the two wandering a Amygdolon's traving on the outside of the fire ring. Terror also drop Cazic Thule Guardian Band other 2 drops that i had gotten are already posted.

Dropped a Glowing Nebulous Dream Fragment, a t3 leg drop, and ring of pure terror for me. Likes to go in circles around the flames in fear.
