This guy seems a bit tougher to get than most instanced nameds. I am breaking my 3 box up, each getting their own mission and seeing when he pops. Of course it is all random but I went 9 missions (3 chars getting 3 missions each) with no spawn. Then going another 6 or 9 missions before spawning him again.
He dropped Toriss' Chain Ring both times so guessing it is his common drop.

Location -22, p347, n130 is correct, to be more helpful in finding him he is located in the center most "hub" room with the six passages leading out/in from it.
He stands near the head of a table on the south end of the room.
You will either see Toriss the Protector or a vault protector in his place.

1) Had a yellow to 83 version, so would also be level 86.
2) Drops: In six kills have had 4 of the standard ring, 1 type 3 slot ring, 1 standard charm, 1 wrist/hand essence.
3) Strange behavior if you sit at the spawn point for the duration from zoning in... a mob at the ph will periodically, every 2-5 min, "flicker" momentarily in and out. Four of us saw this in four separate instance zones and it can be observed every time. It's only for a split second and if you are watching closely enough, you can see the name, Toriss the Protector or a vault protector. Keep in mind what you periodically see flicker in and out has absolutely no bearing on if it's trash or the named spawning at 13:40ish into the instance. <---- completely useless info. I tend to think there are a minimum of these "flickers" that need to occur with someone in the zone for the mob location to actually populate, but that's purely speculation.
4) The rampage mentioned earlier is a wild rampage, not a target ramp, so dont worry about setting a ramp so it doesnt hit your squishy.

He seems to spawn in two versions, one version wild rampages (AoE) and the other version single target rampages.
Spawned him 7 times and 24 placeholders. (So, 7 out of 31 times)
