I was not getting the rare to spawn, so I brought in a tracker and found that A Kerran Ghulam was stuck in the geometry. Brought in a pet class, put the pet on it with little luck, until I agro'd other mobs in the area. That broke the stuck PH loose, to join in the fun.
Update: the PH still spawns stuck in the geometry, so I'm going to have to do this over and over until the Named spawns.

While Toxx is incredibly easy compared to most other hunter achievements in classic this guy for me was the rarest of the Toxx names. I had all the others spawn multiple times while waiting on this one. Easy spot to also get kills for Kerran/Vah Shir slayer achievements. Easy grind to 1000 kills while you wait on the spawn.

On the Cazic server there is [a kerran ghulam] in side the mountain on the right of the entrance tunnel.
Location around n454 p1741 p17 to the right and below the hut
if you use the command /target a, you will be able to target [a karran ghulam]
I was not able to autofire arrows with my ranger. I was able to target with my Shadow night
and send in a pet.
update: I was able to find [a kerran ghulam] and is located outside Karra island on the mountain side.
Location n400 p1300 p172
But if you are doing hunters and looking for all the PH. This is how to get them all.
