I just got him first spawn in from the altar area just after you go in the mechanical door.

I have seen the vassal spawn two times now and both times was in the place of a Broken Skull Acolyte. I am assuming that any Acolyte could be his PH. First time was at the bottom of the stairs in shrine. Second time was in a room near where the Blackhand LT spawns. Hope this helps. Also he dropped bone crafted twice. Was hoping for earring.
