It CAN be click during combat now. Was changed in a recent patch.
Do you mean this mask cannot be clicked while fighting, or that you cannot cast in Dragorn illusion form (when wearing the buff effect)???
Some other questions:
1. Does the Damage Bonus from this do all that much with current level 90 gear?
2. Can it be used on a lower level character? (ie. what is minimum casting level)
3. Is the illusion cancelled when conflicting melee abilities are activated, or simply ignored (ie. do you return to normal form and lose the buff icon)?

This is very rare, not common as listed.
Very common, in fact it is a choice when you log in around the date of this post.
It is only "common" to those with an active LoN account during the time of the SOE security breach. Even at that, the LoN reward is a "choose your own loot" from the Legacies expansion. While I was redeeming mine, there was plenty of chatter from people deciding to go for one of the "set" items for the 25 slot WR bag. Many raid players also opted for the Talendor trophy, as the effect on the illusion buff does not stack with several Berseker or Monk abilities.

Where is this from? I know it's from Legends of Norrath, but I don't know which pack

The latest one Legacies. I got it from the 5 free no trade packs along with the Tekko and Crushing Vermillian Dragon Ornaments. I think I used up all my good luck on that one pack though.
