This link suggests that the one that drops off a warlock is a different box, however I can confirm that after looting from a warlock, I could not loot one off the shaman.

If you notice, the other box drops off "a Bloodmoon warlock" when this box drops off a named "Bloodmoon Warlock". You looted this box off the "a Bloodmoon warlock" and then killed a "a Bloodmoon shaman" and you had a conflicting box lore item on the corpse?
