The "Size" designation on items was added in a recent patch. It refers to the size of the item itself -- the type of container into which it will fit -- rather than the size of the race that the item will fit.
So, a "small" item would fit into a dusty ransacker's pack (which holds small & med size items), but not into a wrist pouch (which only holds tiny sizes).
A "large" item such as a shield would not fit the dusty ransacker's pack, but does fit in a backpack.
And a large sewing kit still has the best carrying capacity pet slot, taking extra large (or whatever they are called) items.
