This particular piece uses the following:

1 HQ folded sheet (1 block HQ ore, smithy hammer, water: combine in forge)
1 sea temper (2 sea foam, 1 saltwater seaweed, fish wine: combine in brew barrel)
1 leather padding (1LQ cat/bear/wolf pelt, 1 silk thread: combine in tailoring kit)
1 imbued rose quartz (but rose quartz from merchant, imbue with lvl 29 cleric spell)
1 plate collar mold (I buy in EFP, upstairs at blacksmith shop near the zone to WFP)
1 smithy hammer (buy in NFP, at blacksmith shop by bard guild)
1 chain jointing (metal rings, water, smithy hammer: combine in forge)

These items MUST be combined in the FREEPORT FORGE. No other forge will work. I succeeded on my first attempt at one of these last night, with a 181 smithing skill, but that may have been luck. It was NOT Trivial.

BTW :

(block of HQ ore = 3 large bricks of hq ore+1 water: combine in forge) Bricks are now supposed to be for sale in the Bazaar on Luclin

Saltwater seaweed is fished in Ocean of Tears. Sea foam drops off seafury cyclops, as well as the regular cyclops that are the placeholder for old AC. Fish wine can be bought at many taverns.

MQ and HQ pelts can be skinned down to LQ, using a skinning knife, which is made using the blacksmithing skill.

Silk thread is made by combining 2 spiderling silks in a tailoring kit. This part never fails, and is the only part of this where you can't lose the raw components if you fail the skill check.

You don't lose the smithy hammer, or the skinning knife, if you fail a skill check. Everything else goes poof.

Hehe, I just realized I probably gave WAY more info than you asked for, but I just made my first one last night, so am pretty stoked.
