Combat Effects actually modify the formula that your proc rate is determined by. Over the course of time, a basic weapon has a proc rate of 100, which will equate to about 2 procs per minute (at level 65 with 305 Dexterity). This item would make that base proc rate go from 100 to 105, and made you proc about 2.1 times a minute. Certain items have a much higher proc rate then 100 as the base, however (Cleric hammers are a good example of this.) Proc rate is also affected by the Weapon Affinity AA.

Accuracy is just a simple improve in how likely your attacks are to hit. The most common formula is using 3 Accuracy to equal 1%. If your normal hit percentage is at say, 60% and you have +12 Accuracy from items, your hit percentage would increase from 60% to 64%.

the formula for accuracy is +15% accuracy to 1%, accuracy caps at 150 worn.

Can somebody point me to a URL or page that talks about these modifiers? Combat effects and accuracy?

Combat Effects are exactly that... Effects that occur during combat (read: weapon procs). The +5 modifier adds to your chance of proc'ing, as if you had a "Proc" skill.

Accuracy is your accuracy, or how likely you are to hit.

"Strikethrough" is an addition to your chances of breaking through an enemy's defensive skills... Like block, riposte, dodge, etc.

DoT Shielding is pretty self-explanatory.

I can't think of any others for now, so that's all I can tell ya.
