Are these boots +20 atk +2 regen straight, now, or is it a worn buff?

It'd be the same as if the item was coded +20atk +2 regen. A lot of old items with the worn aura effects have been changed to direct stats as opposed to the worn description (i.e. belt of the great turtle)

So if you have your atk and/or regen maxed, you won't get any added effects from this item.
