To add an augmentation to an item, go to any of the five adventurer's camps and look for the little "birdbath" on the ground. Put the existing item you want augmented and the augmentation in the birdbath and hit the Combine button. Voila, the augmentation has been added to your existing item.
Removing an Augmentation

If you destory the Adventurer's Stone by removing it from an item, then go back and get another, will it have the same stats as the last one you had that you just destroyed? I would think No, but just thought I'd ask.

It's a charm based augmentation. It has nothing to do with your current Adventurer's Stone... but rather your faction with the Wayfarers. You could destroy it, get a new one, destroy it, get a new one and re-apply it to your charm and get the same increase in stats.

When you put an augmentation into an item, what is the areas it is usable in ? Is it only taking effect once you enter LDON dungeons or is it effective all over norrath ?

Sofar not been able to find any info on it.

Depends on the augmentation. The basic items on the adventure merchants that you can see before you do any adventures are all Charm augmentations. They say "will give you <effect> when you are in a LDoN zone", but that is how charms are... only work under certain conditions. All other augmentations that are for other items (armor, weapons, etc.) are straight up upgrades to your items. So if you add 5 STR to your BP then no matter where you are (LDoN or otherwise) you will have 5 more STR when you wear that BP.

Their is 1 Npc at each camp, if you hail them you will see in Red "You need to go on more adventures before" or somthing along the lines of that.. I did 2 adventures for the EC Camp and got 10 points. Hail the person and say in yellow "You have gain more knowledge about the Wayfarer's Brotherhood" or somthing like. hailed him again and he said he had more to share with me but I needed to do more favors

So it kind works like the Gukta quest.. where you just Hail the NPC instead of handing them the item, and it stores it in your journal.

There are some more posts more towards the top now say how you get additions on your charm in summary more adventures 10 to start off and 10 each bump after that it sounds from there posts

I'm pretty sure it's just that. Learn more about the Wayfarer's. Talk to NPCs, learn stories etc. I don't believe it's adventure based as you spend your adventure points. But I've been wrong once before.

I have found NPCs around the world that if you Hail them will give you a little story line, then say something like, "I will tell you more once you have adventured more in Miragul's Menagerie". The NPC I found was in Greater Faydark just south of the zone to BB.

As I understand it you Hail the Bard at the adventurer camp for the theme you just completed an adventure in (Bastre at Shady camp in EC tunnel for Rujakian hilld theme). Every 2 adventures you complete it will say you gained important information. If you hail the bard and your havn't sucessfully completed at least 2 adventures since last you hailed them you get a message telling you to adventure more.

If you use your LDoN key item to augment a charm, and then remove it, you can get another by talking to the same guy that gave it to you in the first place. At least that was what my npc said when I did the quest. He said that if I ever loss my key, come to him and he would replace it.

Thats interesting.. Because mine is now attached to my charm. :p To attach it to a charm, just click on the little pool of water that is at each Wayfarers camp and it will open up a window. Place the charm and stone in and whammo, you now have an augmented charm.

This is an augmentation for the CHARM slot, not a CHARM item in and of itself. Allakhazam's needs a new item format to show this information properly.

A couple of people have posted that the stone is an augmentation of the charm SLOT. If it truly augments the slot, then it should affect whatever charm is placed there. My understanding is that an augment is placed on an ITEM and that an augmentation may have restrictions regarding the slots that the item being augmented fits in.

Perhaps this is just semantics but it is a key question. If I have 2 different charms, does my adventurer stone affect both (augments the slot) or I choose one and it augments only the one I bind it to (augments a specific item restricted to a slot). My guess is it's the latter. However, I believe I read that you could get the stone replaced by talking to somone in the Wayfarer's camps, so you could theoretically use a solvent (if that is even necessary) and then get a replacement to use on a different charm, right?

No, augmentations affect the ITEM, not the SLOT.

Also, you can remove and augment from an item, but it will destroy the augment.

It only affects the item it is placed in. This augment just happens to be restricted to the items available to the CHARM slot...

can augmentaions be removed, I noticed that port guy at wayfarers camps sells solvents that look like they remove augmentations (destroying them?) can anyone confirm this?

Yep, they remove the stone, opening the slot again but destroying whatever augement the solvent removed.

If you notice by the groups of Wayfarers there is what looks like a Birdbath or table with water in it. Click on this and it will open a 2-slot container. Place your augmentation stone and item in their and click combine. That simple!

I combined my Adventure Stone with my Intricate Wooden Figurine charm from PoK.......They should make for an awesome charm if (or hopefully when) both are maxed!

The Adventurer's Stone that I got is an Augmentation for the Charm slot not a charm itself. Wondering if stats will be the same or what??

I attached the stone to my Intricate Wooden Figurine and the stats improved on it. This was a suprise to me as I figured I would have to do some adventuring prior to any increase in stats.
