Just dropped from Ancient Guardian in Chardok, he is the golem that spawns at the bridge. The Bridge Keeper can spawn in his location also.

Several of these dropped in the Deep when my guild was running the Burrower Beast event. Dropped from one of the many worms that spawn during it.
