Krystale, what did you loot this off of? We kill everything in the Plane of Fire ever few days and don't have a single one of these yet. Did you get this off a bird or off a general? Eqmaps says that they might drop it but their information is often wrong.

Any of the listed classes able to use this will benefit greatly by having this on while in combat. theres lots of pally combat spells.. the yaulp line is one of em, and with this mask there is plenty of mana to yaulp away forever. heheh... And im sure even rangers, shadowknights, bards and beastlords have a line of "melee range" spells that require tons of mana ... mana that no longer needs to be regained after combat... just my 2cp
Ulvian, while you make a valid point (that all listed classes can use this), your underestimation of a paladin's need for mana regen is seriously flawed.
I can understand your confusion, since, from the point of view of a ranger, mana is mostly used for damage output. Your error here is trying to compare the arcane offensive output of Paladins, when pallies are not an offensive class.
If you played a paladin post 55, you'd know that: (1)Pally stuns are used primarily for agro (2)Yaulp only costs 10 mana to cast (3)No paladin casts that useless DD spell (4)Our group heals are awesome (600 mana for 3960hp worth of heals to the party) (5)Our regular heals are pretty good too
This item, in the hands of a paladin, will regen mana needed for group heals and stun-taunts. In the hands of a ranger, it will allow more mana for nukes and dots. In the hands of an SK, it allows more mana for lifetaps and dots. Each class can use this, and each has a situation where its more desirable for one class to have over another.

I can't say for sure but it looks legit to me. There have been several items introduced in PoP with features previously rarely or not seen before, the most prominent example i can think of is the new bard weapons that count as instruments as well (i'm not entirely sure but i think there may have been a few of these before the release of PoP)
As to this item, my best guess, only a guess mind you, is that the free thought on this item only works while auto attack is on. I would imagine it has some restrictions (maybe you have to have a target/be in melee range of it) but by itself it wouldn't be nearly as much mana as medding even if you stood up and left attack on w/o a target.
Even without the effect this is an incredible mask for hybrids, great hp/mana bonuses and not too shabby with the ac and stats either =)
Just my thoughts... please correct me if you know how it actually works, would love to know.
