Had this drop in the factory and gave lvl 53 Paladin. At the time the recomended lvl description was lvl 50. At 53 he could not use. After a recent patch the item has been marked required lvl 55.
Does anyone have any REAL information about this item? Like, for example, what mobs it drops from? Or, is it quested? What area does it drop from? Is this just a random drop from one of the regular mobs?
Please shed some insight if you happen to have this hammer.

One thing that was not mentioned about damage bonus.
First off, mitigation of the NPC is completely negated by the damage bonus. When hitting with that weapon in your primary hand, the damage bonus (+1) is your minimum hit.
Note: primary hand. The off hand does not get that damage bonus when you dual wield. There is *SOME* damage bonus applied if an off hand weapon has high delay (e.g. longsword of ethereal energy appears to have a 1 or 2 point damage bonus) but it is nothing like the mainhand damage bonus and it is not displayed; it can only be detected by observing your actual minimum hit.
I want to dispel this myth before it infects anyone else. The offhand has no damage bonus. None. That's why you put the slow weapon in the offhand, so the faster weapon has the damage bonus applied more often.

Isn't damage bonus based on level? And 2-handed weaps get more damage bonus than 2-handers, I do believe. It's just the lowest amount of damage inflicted, without missing. So if you swing and nick em for 1 pt at low level where there is no damage bonus, that same would do 41 at the appropriate level (guessing 60's). You're mostly right. The damage bonus is different, whether you're using 1handers or 2handers (bows have no damage bonus, even though there's one listed on the item). 1handers have a specific bonus based solely on level. Whether you have a revultant whip (5/14) or a wurmslayer (25/40), the damage bonus is the same. On 2handers, the damage bonus is determined by two factors: level and delay. At any given level, weapons with a higher delay have a higher damage bonus.
Now, for how a damage bonus actually works. A weapon is swung, if it's a hit, the damage is applied. After the initial damage is calculated, the damage bonus is added. So, for example, let's say you took two swings of this weapon, and connected both times. The damage is calculated as 100 for the first hit, and 1 for the second hit. Add 40 to each, and you get 140 and 41. That's how the damage bonus works. The damage bonus is *always* added, so long as the weapon connects, and it's always the same amount. Damage bonuses work the same for 1handers as they do for 2handers.
This sounds to be about right, the only thing missing in the scenario is damage mitigation, the ability of the creature to lessen the amount of damage taken. This can be accomplished by various means, armor or hide, abilities etc., this explains the low damage numbers that are seen at times.
Hmm .. if your explanation of the damage bonus is true, then how do you account for the ever common hits for 2 and 4 and 1 damage, even at level 60 with a damage bonus of 11?
Damage bonus "should" work the way you described it, but I don't think it does, unfortunately. I've tried to figure it out, but I cannot find a formula that truly explains it.
Damage bonus is only applied to primary, so when dual wielding, you'll eventually have a few very low hits, coming from your off hand weapon, even at 60.
And mitigation doesn't interfere with minimum damage, if you have X damage bonus, you'll hit at least for X+1 each time you connect.
