As of December 2020, Scalp of the Ghoul Lord is confirmed to still drop. It appears to be an ultra rare drop as my character went all the way to +2000 faction only faction farming in Lower Guk at +1 per kill. All undead named were killed along the way including the Ghoul Lord. After hitting +2000 faction it took an additional ~8 hours singularly camping the Ghoul Lord for this to finally drop.
Killed Lord while factioning. Read the scalp. Thought I was out of luck since I'm agnostic. Tried it on anyway for the heck of it. Took me from amiably to kindly. No idea why. Might be bugged or something, but worked.
Am able to wear this as a Human Warrior of Rodcet Nife, the item gives no stats when worn, has the yellowish look when worn on the inventory, but the faction mod works just fine. Took me from Kindly to Warmly.
was clearing lower guk on my friend's beastlord and got lucky, lord was up and dropped this. actually took her from a high dubious to an amiable. that is quite a faction boost.
regarding below post- the beastlord is agnostic vah shir, and scalp can be worn and the modifier works. apparently the race/diety restrictions are not in effect. unsure about class

As a 67 rogue of the Bristlebane deity, I am Indifferent to the frogloks when I am not wearing the scalp, but when I put it on my faction changes to amiable. Anyone else have this happen to them with a deity that is not listed?
Call me lucky, but i got it on my first try with a 74 bst. I had a PH up when i first got to his camp. Singled pulled and murdered the Bok's. Fight took less then 1 minute. He is not as hard as he used to be.
Was fun getting down to guk again for the clicky woody crown! Still killing for faction unfortunally *crys*
I killed the SOB 6 times in 8 hours and he still hasnt dropped the damn scalp. Oh how i wish i hadn't deleted it. Does anyone know if you need the scalp to MQ a reet crown? Probably so. Now i want the damn thing just on principle.
Bards dont need to use this anyways(But it would be nice :) ), with their level 11 Cinda Charismatic Carrilon song, it took me from dubiously to Amiably, Then i did gnome illusion but it didnt go up, still need to try feral pack.
You can get lucky on this drop. I came to the Ghoul Lords room and killed its PH. Next popped the Ghoul Lord, no scalp, just a Skull Shaped Barbute. Then a PH again, and then the Lord again which now dropped both a Barbute and the Scalp!
Unfortunately the scalp does not much for me yet, I am almost max KOS to the frogs, being a Cazic Thule Troll that has hunted Guk in his youthfull days. However, I think when I can get my faction to Indifferent by killing a million frogs (I think thats the highest us Evil Cazic worshippers can get when it comes to bare faction)this scalp will lift me up a good deal since going from Indiff to Warmly is a much shorter stretch then the dreaded "scowling" and "dubious" stretches. Not sure if it will take me all the way to warmly though.
Wolfform (tested on outside froggies that guard the entrance) did not un-KOS me either, but it did take my druid from Dubious to INdiff. So I am still unsure where Wolfform comes into the whole equation for evil races.
I guess the first thing I have to do is max my faction by killing (Your faction could not possibly get any higher.) and then see what all the illusions and the scalp do.

It's bad enough that us darkies got screwed in that a quest for an item that would be best for US involves getting faction with one of the races we are all garanteed to have crappy faction with.
But just to add insult to injury, the only thing that will allow us to do this quest is apparently an insanely rare to boot!
Some ogre BL was camping it the other night with a full blown AoE group - they were clearing entire sections at a time without issue. They managed to spawn the Lord 12 times and he didn't drop the scalp once.
I got hold of the camp after they were done and four spawns later, still no scalp (and a darned tought fight too - this 47 vs my 54 and he nearly wins most of the time thus far).
Camped him for about 3 hours today for faction. In those 3 hours we killed him 3 times and everytime he dropped this scalp along with a skull-shaped barbute. Could be the droprate has been changed though as it seemed to me it was his common.
Yeah, killed the Ghoul Lord once last week and it dropped the scalp. Let it rot since we didn't know what it was for then... gonna have to go back at 61 and tear the place up, I want to be a Wood Elf Necro :).
If I kill some Dwarfs and get one of their scalps and put it on my head, will I be able to walk through Kael untouched? I figure the giants will look at me and say, Giant 1: "Lets kill this Dark Elf! Oh wait whats that on his head?!" Giant 2: "Looks like the scalp of our most hated enemy, the Thurgadin Dwarves" Giant 1: "Well in that case he is cool, leave him be." Me: "Hey guys look whats on my head a Thurg Dwarf scalp. So we cool right? We are best buds? No hard feelings right even though I killed plenty of you guys before?" Giant 1 & 2: "Yup you may pass freely Dark Elf" Me: /bow. Thurg Dwarf scalp falls on ground and I pick it up and try to put it on like it was a toupee. But the scalp won't go on straight. Giant 1 & 2: "GET HIM!" *Start Benny Hill Chase music* Me: /runs to Wakening Lands Zone line. "Train to WL!" /scalp falls off from running. THE END!

Frogloks have always been a "good race", they didn't come up with all that "Mithanial" stuff for LoY. Dark races, classs, and gods are not welcome un upper guk without a huge faction rehaul.
However, there was a quest added. The crown of the deciever requires a LOT of upper guk faction, and it would be unfair to make the darks work twice as hard for it. So, they added an item with a faction modifier to a high level undead frog to help out. Voila.
Probably because barbarian + rallos zek doesn't make your hard faction modifier as low as being an evil race + rallos zek. Thus your faction cap is high enough to do the crown of deceit quest w/o the scalp
One faction I know it raises is live side frogs. I got this just the other night and we were clearing from frenzy to lord so i ran back to frenzy room. Without this on, apprehensive to the live frog in the cage in frenzy room, with this on amiable.
This is in fact the only faction it modifies. I don't know why everyone else who posted on the page is so mystified by that. Just use common sense, it dropped of the leader of the undead frogloks, the arch enemies of the live frogloks, who do you think would be happiest to see proof you killed him? Duh. Identify does show this by the way.
The main purpose of this is to help the followers of evil deities, who have worse faction with the live frogloks, to be able to get the crown of deceit. Keepin minsd it won't get you from KOS to ally by itself, you're still going to need to kill TONS of undead frogloks to ever do the quest. But this is a nice boost in the right direction.
Just for those who are confusing the two: the deity restrictions and faction modifier are TOTALLY UNRELATED. They may be related in concept (e.g. the item designer may have put them both there for the same reasons), but the effect of faction modifiers has nothing to do with the deity restrictions.
So, to the person who guessed that this would raise your faction with PoH residents, that's quite possible. We don't know the faction that it modifies, so we have no way to know for sure. My guess is that it raises faction with all of the dark races. Again, just a guess.
PoH residents? Dark races? What are you smoking anyway?! Even for a guess, it's a dumb guess. This raises faction with Frogloks of Guk, which if you thought about it, makes perfect sense. The live frogloks hate the undead ones, and this drops off the leader of the undead frogloks. Even if you didn't just cast identify, using common sense would allow you to figure that out. Why anyone would think it would raise faction with Plane of Hate residents is beyond me. And most of the people who can wear this are likely to be a dark race anyway.
Thank you, Troodon, thank you! Thank you for trying to spreak a little sanity/brain power into this dim-bulb world! For anyone's information, items that have a "faction modifier" only modify one faction -- the faction found in that zone (or, a faction, if, like Lower Guk, there is more than one faction for all of the inhabitants). The Planar Traveler's Manual, or Amulet of the Haven do NOT give you better faction with the Inhabitants of the Plane of Hate. They give you better faction with the inhabitants of the zone in which they are earned. The same with the Scalp. The only differences being, that 1) it is looted instead of earned, and 2) it modifies the other faction than the one from whom it's looted.
It should be similar to the Amulet of the Haven item in Shadowhaven. Without my Amulet of the Haven equipped, merchants are dubious and will not sell to me, with it equipped they are magically indifferent to me. You must be a follower of the deity/deities listed when an item has a deity/deities listed on it. I have never seen an item with more than one like this though...
Hehehe, I have completed the quest in Shadowhaven so many times that I am Ally to them with out wearing the Haven Amulet. =) It only takes about 2 hours and 15 rounds to do it but worth it if you dont want to have to carry the Amulet with you to sell and get deacent prices.
That amulet was pretty worthless to my druid... he did the quest *once*, and just turning in the quest items adjusted faction from apprehensive to amiable by the end. Equipping the amulet didn't do anything at all for his faction. Since then, he's done the quest a few times, and destroyed the amulet.
Also, I have two characters that have done the quest in Shadeweaver's Thicket for the Blessed Champion's Cape. It supposedly modifies faction with the Guardians of Shar Vahl... but you need to have amiable faction to even start the quest, and after completing the quest and equipping the cape, I didn't notice any change in faction.
Maybe I was just barely into a faction "title" and the modifiers were there, but not enough to reach the next faction "title". Maybe the faction modifiers were broken back then, and they're working now. Who knows. However, if this thing works for you, and allows you to eventually turn into a wood elf monk or shadow knight and amaze your friends and newbies alike, then good for you.
