My wizard level 3 is Formulaic weighted tabard.(6450 hps and mana, 5 luck).

I understand that you are having trouble with figuring out the Threadbare Weighted Tabard. Our ability to provide gameplay help or walkthroughs is limited from here in in-game CS (Especially with newer content), I would suggest consulting with the community either in-game or on our official forums to gain a better understanding of gameplay content. We are not necessarily able to discuss game mechanics or design decisions and we generally urge players to discover new game features on their own or as part of the community, either through the various EQ fan websites or through our official forums:
But if you feel as though something is not working properly, I highly recommend submitting a bug report from in game. Bug reports can be made in game by typing the command /bug and then filling out the appropriate information.

if you did those five and your threadbare weighted is maxed, then right click it to view the stats, then click 'Claim Reward: Weight Tabard and a reward window will open and then select Claim to get it. if you did not complete your collections, then you will not get the claim button, but the inspect and preview. if you are certain you did all the collections requirements, then file a petition stating such.

I have all of the following completed and stil at Zero
1) Call of the Forsaken Master Scavenger
2) Master Scavenger of the Darkened Sea
3) Master Scavenger of the Broken Mirror
4) Empires of Kunark Master Scavenger
5) Ring of Scale Master Scavenger
Only missing 18 or so to finish off TBL Collects. Any idea why it wont evolve to next level?
1) Call of the Forsaken Master Scavenger
2) Master Scavenger of the Darkened Sea
3) Master Scavenger of the Broken Mirror
4) Empires of Kunark Master Scavenger
5) Ring of Scale Master Scavenger

In order to be able to evolve your item, you need to COMPLETE ALL the achievements before being able to level them... ie. you cannot be missing any of the required collectibles.
Levels 1 and 2 matched what was shared, but level 3 is Fused Weighted Tabard for me.
