Do not waste your time trying to get this item in the era of Luclin on TLP servers. The "a pile of fleshless bones" NPC is not in game until presumably much later in the servers progression. I wasted my time camping Praetorian Myral to get to the point of finally turning in the items to sadly realize this. Please do not make the same mistake.

mischief is currently on SoF and a pile of fleshless bones is still not there. looking at the time line it was added during tmb. it might not be available till then. at least i didnt waist time camping the drops. just noticed i had both and went to turn it in.

Head over to Grieg's End and you will need to kill Praetorian Myral (as indicated by 1 on the map), you will need the Scythe of Sundering (Optional- you only need it if you know no one with it that can proc it and turn in the Flesh to spawn the shade for you) and Divine Hammer of Consternation from him. You can tell what he will drop as he will equip and display both of these weapons.

With the Scythe of Sundering you will need to melee proc it on mobs until you have up to 4x Chunk of Flesh, gotten from procing the scythe. (Optional- you only need it if you know no one with it that can proc it and turn in the Flesh to spawn the shade for you)
After this head to the fleshless bones in Mons Letalis (as indicated by square on the map). This is inside the underground cave part.

Turn in the 4x Chunk of Flesh to the fleshless bones. This will spawn a shade npc, turn in the Divine Hammer of Consternation to it and you will be given the Corrupted Hammer of Consternation.

DA hammer NPC in Griegs end
Named is on a 40 min to 8 hour repop. No placeholders. Seems to pop on the hour mark.
2 main drops. Scythe and Hammer. You can tell which he has as he will wield them.
The Scythe is used to get the 4 flesh needed to spawn the hand in mob in Mons Letalis. However you can Multiquest this with somebody else handing in the flesh to spawn the mob. So really only need the DA hammer if you know somebody who has scythe already.
Can stand in the SE corner of the room without agro and watch for him to pop in front of the alter.
Be prepared for a boring wait.
