How to use this:

Equip the Rusted Locket in your primary slot, target yourself, then cast Identify (using spell, AA or clicky) a few times.

After repeatedly casting Identify on the Locket, a message eventually appears:
The softest touch of Tanaan's force after a steep in the Water of Flame.

Combining the Locket with Firewater in a brew barrel results in the message:
The Firewater bubbles on the surface of the rusted locket.

Combining the Rusted Locket with a Tanaan Smithing Hammer in a Taanan forge in PoK results in the message:
You strike the rusted locket with your hammer but nothing happens.

Repeat the process of brew > forge > brew > forge a number of times, eventually the forge combine results in the message:
You gently tap the still bubbling locket with your hammer and watch as it opens.

Receive: Opened Locket.

After literally dozens if not hundreds of attempts to get this thing to proc for my monk, I noticed it could be Traded, so I got an enchanter to try it out. He equipped it and cast Identify (on himself, holding nothing) and it procced several times in a mere 10 casts, giving an emote that described (in riddling fashion) how to open it.

The enchanter also tried the Identify AA, and it also procced more than once in 10 tries, though it seemed less often than the spell.

I took the item back and again tried both the AA and a clicky (Holgresh Grand Vizier Beads) several more times, but neither would proc for me. My guess is that the item mostly procs for the spell, and that casters have some sort of bonus, analogous to Weapon Affinity for melee, that adds to their proc rate on the cast and even adds to the base 0% proc rate for the AA. Oddly, the enchanter could never get the clicky to proc, either.

Anyway, if you're a melee wondering how anyone ever figured out what to do with this locket, the secret is to get a caster to help!

Upon repeated reading of the letter, first:

You get the feeling that there is something stuck inside the envelope of the letter.

and eventually:

You shake the Tearstained Love Letter's envelope until a small pendant falls out.

I tried giving it to Moore again to see if he could elaborate, but:

Archivist Dominic Moore nods as you recount how the locket fell out of the envelope. He holds the pendant up to the light and frowns, 'The clasp is rusted shut. If you could find a way to open it, it would be a more significant addition to the relics in the archive.'

Given that it had taken repeated reading to obtain the locket, I tried repeatedly asking Moore, and repeatedly casting Identify (spell and AA) with the locket held or equipped (though not, alas, both). I gather a brew barrel is involved, so I may go try one to see if I get a clue, but I don't see anything else to direct me there as a way to open this rusty thing.

p.s. Why does it look like a glowy yellow hemisphere?
