Quite often, you will have to run many more than just three heroics to complete a set. You will see this in TDS as well.
Actually, if you manage to get all eight items of a set within 12 spawns, then you are pretty lucky. Otherwise, you just have to keep repeating them or hope they show up in the Bazaar for a price you can afford.

This is from the shining lights that show up half way thru the HA. Not sure you can pinpoint where it spawns as those are random spawn locations and random spawn items - in other words there is no certainty which light contains which collection item. Keep at it since it is not that rare - I have quite a few. Don't forget to check the baz or ask in chat.
