Dropped from a servant of shadow in the maze. Also found one upstairs, not sure where. Not in any collection set at this time.
I just had one drop from a servant of shadow in the curtain maze as well.

I forgot the name, but there's an achievement for completing all collections from PoShadows and Chelsith. In that list there are also 2 individual items. One of them is Luclin's Spy and the other is a drop from Chelsith.

One dropped off trash mobs while camping in the Darkfire Islands in the Plane of Shadow.
Too bad I didn't catch which type of mob it dropped from.
Possibly a zonewide rare drop?
