PH - in the fort in the cavern where the injured wolf spawns. In the middle of the fort is a circular building, he spawned at the single-spawn on the left inside of that.

I have heard of it spawning in the castle too (unconfirmed by me) but I found it as rare spawn in place of one of the luggald zealots or fanatics inside the fort. The picture above is obviously one taken inside the fort.
I have killed luggalds there for a total of about 8 hours trying to get those damn drops for my new 44 mage spell and have seen it spawn 2 times now.

If I'm not mistaken, the stated level range should always con dark blue to a level 53. At the time that I killed him that was my mage's level and this mob conned light blue. So I'd say that he comes slightly weaker than this. He didn't seem any harder that a luggald zealot really either and they are listed at 33-37 and con light blue and green.
He also dropped the Sleek Scale Cape for me.
