The Lavastorm Mountains make for a treacherous terrain, so watch your step as you make your way through the area, being mindful of some signifcant falls.

Not long ago, some miners broke through to the dragon nesting grounds of northern Antonica. These Broodlands can be reached through a network of caves in Lavastorm's north. Also in these caves are two factions of adventurers, each offering expeditions and tasks into the Broodlands. They are Norrath's Keepers and the Dark Reign, and they strictly oppose one another.

This mountain range is also home to many other places of interest: Najena's dungeon is found in the southeast while Nektulos Forest lies in the south. In the cliffs along the lava lake is the entrance to Nagafen's Lair while Solusek's Eye, named after the lava lake and home to the Goblins of Fire Peak, sits in the west.

All the while, there are swarms of drakes, fire imps and elementals, goblins, dervishes, basilisks, undead, and lavaspinners roaming the area. Also, beware the lava dragon, Tirranun, who has been known to make occasional appearances around the lava lake. Of course if you hear him approaching, it's probably already too late...

Pain and Suffering

I too have fallen victim to this misteriuos Pain and Suffering. Back when I was lvl 8 in Buthcher Block. I was fighting a Gob Shaman and he DOT'd me, the battle went south and I ran to guards at x-roads. The guards killed the Shaman, and I hid in the house. I had very little HP left and I saw the pain and suffering messages, killed me dead. So apparently it is in multoiple zones, and has something to do with DOT's.

Pain and Suffering exists in every zone (you just need to find it). Rather than invent some other method of killing you at extremely low hp/DOT'd, they made the invisible mob Pain and Suffering. Not really meant to be targeted or killed, which is why it takes improvision to target it. Since it is still technically a mob, it would take extra editing to filter it out of aoe spells and track.
Another easy Pain and Suffering to find is the one in Paineel on the ledge in the newbie area.

Look at Amazonia's post above and you will see why u can be hit by this "pain and suffering" in any zone. It is simply damage being done to u when u are either unconscious or bein hit by a DoT. Other than this zone, i dont think there are other mobs liked this one.
