The eastern plains of Karana are sandwiched by mountains and water. Travelers should be wary of the named cyclopses, wandering evil eyes, and steep, narrow mountain paths approaching Highhold Pass (don't step off the ledge!).

Its a bugged mob...usually ends up with the ! and has very few HP

A mob that is called ! (or has a ! in front of the name) is a permakilled mob. In short, yep, it was badly bugged and needed fixing. It's easier to just change the NPC's name than it is to completely remove it from the zone.
