 Loot a Burden of Truth from a Tier 1 or Tier 2 raid (rare drop) and evolve it to its highest level.

Once it reaches its highest level, a reward window pops up. You are offered a class-specific helm item:

Bard - Freedom of Charisma
Beastlord - Freedom of the Wild
Berserker - Freedom of Rage
Cleric - Freedom of the Righteous
Druid - Freedom of Nature
Enchanter - Freedom of the Mind (please upload to Lucy)
Magician - Freedom of Knowledge
Monk - Freedom of Spirit
Necromancer - Freedom of Damned
Paladin - Freedom of Fortitude
Ranger - Freedom of Alacrity
Rogue - Freedom of Dexterity
Shadow Knight - Freedom of the Power
Shaman - ? ? ?
Warrior - Freedom of Strength
Wizard - Freedom of Thought

Looks like the quest itself is called An Uncursed Helm

I got mine from the Kael raid.

And it pops up as a quest reward as soon as you get the Burden to 20. No right click, no hand in, the Burden of truth just disappears, and a Quest Reward window pops up with the class hat for you.
