Upon looting the Shrunken Head from Hatchet the Torturer in the Demi-Plane of Blood, you will see this:

As you touch the Shrunken Head, you momentarily gain an understanding of the Demiplane of Blood. Perhaps if you showed it to the right individual in Dreadspire they could help you understand more.

The right individual is Ur-Koraag. Approach him with the head in your inventory.

You say, 'Hail, Ur-Koraag'

Ur-Koraag says 'You have trophy, Shrunken Head. That from Hatchet I see. Means you have Ur-Koraag respect. Strong are ______. If share with me the trophy, Ur-Koraag share with you tale.'

Give him the Shrunken Head.

Ur-Koraag traces a clawed finger along the curves of the Shrunken Head. 'This no longer remnant of a being. To those living here, this badge. Trophy. Mortal life mean nothing to those who make Dreadspire their home. Lives are used, like tools. Thrown away when no further needed. Why that so? Could be blood drinkers [forget] what it like to be mortal. Or maybe they hold no care toward life to shows [superiority] to others. Or now that [mortals is food], it follows that they is also tools. Maybe they just [angry].'

You gain experience!!

There are four levels of the Aura of Crimson Mists zonewide AE in the Demi-Plane of Blood. Turning in the head reduces the effect by one quarter.

Ur-Koraag squinches his face tightly, lost in thought, 'That make no sense, ______. Me know angry and blood drinkers no mad. They malicious. They selfish. But they no angry.'

Don't do it. Ur-Koraag will assist the other trash and kill you, rather than accept your turn in, if you try to train to him.

The hand in mob is Ur-Koraag he is located where Hatchet's chamber door would be in Dreadspire Keep.

shrunken head drops off of an orc manservent near sisters. Got two tonight.

you are not talking about the same item.. you dont get hatchet blocker from trash mobs.

All 4 tier 1 blockers DO drop rarely from the appropriate trash type: ZT one from the gargoyles that turn you to stone, the Redfang one from bats, the Hatchet one from Orcs in the castle area, and the Sisters one from female vampire types in the castle.

The heads do drop off the orc manservents. Whether they are supposed to or not..our guild got two last week.
